Profhilo is the skin booster we are asked for by name more than any other, and the one most often confused with filler. It is not one. It is a high concentration of pure hyaluronic acid, injected at a handful of points and left to spread through the tissue, where it hydrates the skin and prompts it to make its own collagen and elastin. The result is skin that looks firmer, smoother and better hydrated, with no change to the shape of your face.
It suits a specific problem: skin that has lost quality and firmness rather than volume. Someone who has been told they need filler and does not is very often the right person for Profhilo.
Profhilo tends to suit you if- Skin on the face, neck, décolletage or hands has lost firmness and looks dull, crepey or tired
- Early laxity along the jawline or lower face, where you do not want volume added
- Fine lines that come from skin quality rather than from expression
- You want an improvement that people notice without being able to name
- You are maintaining skin quality alongside, or between, other treatments
- The concern is lost volume or a change of shape — that is a filler question, and we will say so
- The lines you want treated are made by expression rather than by skin quality — a different treatment

How Dr Haroon Ashraf approaches Profhilo
Assessment
We look at skin quality separately from structure: the texture, hydration and firmness of the skin, how it moves and recoils, where laxity is starting, and whether what you are seeing is a skin problem or a support problem. The two are treated differently and are constantly confused, which is how people end up with filler they did not need, or with a skin booster where filler was the answer.
We also look at the neck, décolletage and hands, which age as visibly as the face and are usually left out of the plan. Profhilo is one of the few treatments that does something useful for all three.
Product and Technique
Profhilo is a stabilised hyaluronic acid at one of the highest concentrations of any injectable, and it is not cross-linked, which is why it spreads through the tissue rather than sitting where it is placed. On the face it is injected at five specific points on each side, the bio-aesthetic points, chosen because they sit away from the major vessels and let the product diffuse across the whole area from a small number of injections. The neck, hands and décolletage each have their own map.
The product itself is completely absorbed within two to three weeks; what remains is the change it has prompted in your own skin. It adds no volume, so there is nothing to look overdone.
Comfort
Numbing cream goes on first. The face takes ten injection points in total, a few seconds each, and most people describe a sharp sting rather than pain. The whole thing is over in five to ten minutes.
Small raised bumps at each point are normal and are the product itself, not a problem; they can last from a few hours to, sometimes, a day or two, and are significantly less visible after the first 24 hours. Bruising is possible, as it is with any injection.
What the appointment looks like
Before You Arrive
You will be sent a medical questionnaire and consent form to complete beforehand, so the time in clinic is spent on you rather than on paperwork. Come without make-up on the area if you can.
Ideally, avoid alcohol for 24 hours before your appointment. If you take any blood-thinning medication, let Dr Ashraf know — continue taking it as prescribed; it simply means bruising is a little more likely and we plan around it.
On the Day
Plan for around 30 to 45 minutes. Consultation and skin assessment come first, then numbing cream, then the injections themselves, which take five to ten minutes. You can go straight back to your day.
We photograph before and after at every appointment. Those photographs are yours, they form part of your clinical record, and they are only ever used elsewhere with your separate written permission.
Afterwards
Expect small bumps at the injection points for a few hours to, sometimes, a day or two, significantly less visible after the first 24 hours, and occasionally a bruise. No make-up on the area for the rest of the day, and no strenuous exercise, saunas or heat for 48 hours. The first change is usually visible at around four weeks, the second and, where needed, third session, four weeks apart, complete the course, and the full effect is at around three months, because you are waiting for your own collagen rather than for a product.

Risks, and how we reduce them
Profhilo is a medical procedure and it carries medical risk. Less of it than filler, but not none, and any clinic that tells you otherwise is not one to be treated at.
Small raised bumps at the injection points, lasting from a few hours to, sometimes, a day or two, and significantly less visible after the first 24 hours. Mild redness and tenderness. Occasional bruising.
Bumps that take a few days to settle. More noticeable bruising, particularly on the neck and hands. Temporary swelling. A mild reaction at the injection sites. All settle on their own.
Infection, and hypersensitivity to the product, are rare but possible with any injectable. Both are recognised early and managed properly by a doctor who knows what he is looking at, which is the point of having one.

Profhilo FAQs
How is Profhilo different from filler?
Filler is cross-linked hyaluronic acid that holds its shape where it is placed; it adds volume and structure. Profhilo is not cross-linked, so it spreads through the tissue and hydrates and remodels the skin without adding volume. Filler changes contours; Profhilo changes skin quality. Many people benefit from both, but they are answers to different questions, and the assessment is about which question you are asking. More on our dermal filler treatments.
How many sessions of Profhilo do I need?
Two to three, four weeks apart, is the course. Most people have two; some skin benefits from a third. After that, a single maintenance session when the effect begins to fade keeps the result without starting again.
When will I see results?
The first change, mostly hydration and a subtle glow, usually shows at around four weeks after the first session. The firmness comes later, because it depends on your skin producing new collagen and elastin, and the full effect is at around three months after the course is completed.
How long does Profhilo last?
Typically nine to twelve months, and significantly longer with maintenance. Like every skin treatment, it is working against time rather than stopping it, so the honest answer is that it lasts as long as you maintain it.
Can I have Profhilo on my neck, hands or décolletage?
Yes, and these are the areas where it earns its reputation. The neck and hands are the parts of the body that give age away most reliably and that filler does least for. Each has its own injection map, and a session covers the face, hands, neck and décolletage together.
Does Profhilo hurt?

Can I combine Profhilo with filler or anti-wrinkle treatment?
Yes, and it is often the right plan. Profhilo treats the skin; filler treats structure; anti-wrinkle treatment treats expression lines. They address different things and are commonly combined, with the order and spacing decided at your consultation. Where the skin is sun-damaged or acne-scarred rather than lax, polynucleotides may be the better skin treatment, and we will say so.
